may be made available by end-users through a Communication Service, ("Software") is the copyrighted work of Physician
Primary Care Plan™ and/or its suppliers. Your use of the Software is governed by the terms of the license agreement, if any, which
accompanies or is included with the Software ("License Agreement"). You may not install or use any Software that is
accompanied by or includes a License Agreement unless you first agree to the License Agreement terms. For any Software not
accompanied by a license agreement, Physician Primary Care Plan™ hereby grants to you, the user, a personal, non-transferable
license to use the Software for viewing and otherwise using the Physician Primary Care Plan™ Web Site and service in
accordance with these Terms of Use, and for no other purpose provided that you keep intact all copyright and other proprietary

All Software and Applications are owned by Physician Primary Care Plan™ and/or its suppliers and are protected by copyright
laws and international treaty provisions. Any reproduction or redistribution of these Software or Applications is expressly

that makes it available, are free of viruses or other harmful components. Physician Primary Care Plan™ does not warrant or
represent that the use or the results of the use of the Physician Primary Care Plan™ service or the materials made available
as part of the Physician Primary Care Plan™ service will be correct, accurate, timely, or otherwise reliable.
You specifically agree that Physician Primary Care Plan™ shall not be responsible for unauthorized access to or alteration of
your transmissions or data, any material or data sent or received or not sent or received, or any transactions entered into

Termination
Physician Primary Care Plan™ may terminate your access to any part or all of the Physician Primary Care Plan™ service and
any related services at any time, with or without cause, with or without notice, effective immediately, for any reason
whatsoever. If you wish to terminate your account, please contact Physician Primary Care Plan™ . Physician Primary Care Plan™
shall have no obligation to maintain any information in your account after termination.
Modifications to Terms of Use
Physician Primary Care Plan™ reserves the right to change the Terms of Use or policies regarding the use of the Physician
Physician Primary Care Plan™ service at any time and to notify you by posting an updated version of the Terms of Use on this website.
