Physician Primary Care Plan™ (PPCP™ or My Physician Plan™) offers individual self-pay plans starting at $49.95/month for 12-month plans. Members can choose a short-term plan (1-month plan, 3-month plan or 6-month plan), or a 12-month plan based on their needs. Discounted family plans are also available.

Plans for Individual Members:

  1. PPCP™ 12-Month Plan: $599.40 for the twelve-month term ($49.95/month), unlimited visits, $30 copay at each visit.
  2. PPCP™ 6-Month Plan: $350.04 for six-month term ($58.34/month), unlimited visits, $30 copay at each visit.
  3. PPCP™ 3-Month Plan: $250.02 for three-month term, unlimited visits, $30 copay at each visit.
  4. PPCP™ 1-Month Plan: $100 for one-month term, unlimited visits, $30 copay at each visit.
  5. PPCP™ 6-Month International Visitor Plan: $359.70 for six-month term ($59.95/month), unlimited visits, $30 copay at each visit.
  6. PPCP™ 3-Month International Visitor Plan: $199.95 for three-month term, unlimited visits, $30 copay at each visit.
    7. *PPCP™ short-term plans (3-month or six-month plan) and International Visitor Plans—full payment must be made at the time of plan enrollment.

Plans for Families:

  1. PPCP™ 12-Month Family Plan: $1078.80 for the twelve-month term ($89.90/month for 2 members), $44.95/month for each additional member, unlimited visits, $30 copay at each visit.
  2. PPCP™ 6-Month Family Plan: $650.04 for the six-month term ($108.34/month for 2 members), $54.17/month for each additional member, unlimited visits, $30 copay at each visit.
  3. PPCP™ 6-Month International Visitor Family Plan: $659.40 for the six-month term ($109.90./month for 2 members), $54.95/month for each additional member, unlimited visits, $30 copay at each visit.
    4. *Payment installment options are not available for the Short-Term plans and International Visitor Plan—full payment must be made at the time of plan enrollment.
